I finished the game but I see that I have a long way to go in terms of progress...

You can find H replay on Extra tab. also, you need to defeat all 6 bosses on same difficulty(ex : easy 6 times or normal 6 times unlocks H replay, but easy 2 times normal 4 times will not unlock H replay)

When you use [Numb-] key or [1] key, you will see the console command screen on the bottom left screen. then type the cheat code. if you put right code, you will hear the bell ringing sound :D

For some reason, in stage 3, during the survival segment while the elevator powers up - Once you interact with the elevator at the end, it just boots you to the title screen every time.

After 4 times running through this, I figured it was unpassable.

I had the same problem, but it fixed itself just by closing the game and trying again.

More specifically, the bug occurred after dying in the survival mode once, and then upon coming back whenever I picked up turret ammo it was invisible. I assume there was some kind of malfunction in the ammo coding that caused the game to hard reset upon entering the elevator.

3ステージ目で、非常扉を全部閉じた後に動力源の隣のドアを開けようとしてタイトルに戻った形です
扉前で↑を押した瞬間にタイトルに戻っていました

難易度はVery Easyで、全裸瀕死状態だったかな？
